The primary purpose of the LPN Wound Care Nurse position is to provide wound care prevention and treatment to residents and to assure resident safety under the direction of a Registered Nurse, attending physicians and/or Medical Director and consistent with established wound care protocols.

Committee Functions

·        Participate in Interdisciplinary Care Plan Team to develop a comprehensive care plan for residents.

·        Serve on the Quality Assurance Performance Improvement (QAPI) Committee to assess the facility wound care status and improvement plan.

·        Attends morning clinical meetings.

Personnel Functions

The primary purpose of your job position is to provide direct nursing care to the residents, and to supervise the day-to-day nursing activities performed by licensed practical nurses and nursing assistants. 

JOB DUTIES & RESPONSIBILITIES: 

·        Organize and assign all jobs to be done on tour of duty so that workload is evenly distributed among nursing personnel on the basis of staff qualifications, size and physical layout of the facility.

·        Periodically review department policies, procedures and job descriptions and make recommendations for revisions.

·        Ensure that all nursing personnel assigned to you comply with the written policies and procedures established by this facility.

The primary purpose of your job position is to assist the Director of Nursing in determining the staffing needs of the nursing department and to ensure that the required number and type of nursing personnel are available 24 hours/day, 7 days/week and to assure resident safety.

JOB DUTIES & RESPONSIBILITIES: 

Administrative Functions

·        Review the staffing budgeted patterns of the nursing department. Review staffing model.

·        Maintain a daily staffing pattern consistent with budgeted LHPPD.

·        Participate in facility surveys made by authorized government agencies as requested by the Director of Nursing.

·        Receive schedules from RN Team Leaders and LPN Medication Managers who self-schedule and integrate into the staffing schedule for the entire department.

JOB SUMMARY: 12pm-8pm $13.00-$16.00

 The primary purpose of the Dietary Cook position is to prepare food for the residents at the facility and to assure resident safety.

Dietary Service Functions

·        Prepare meals according to planned menus.

·        Prepare and serve meals that are tasty and appetizing in appearance.

·        Serve food in accordance with established portion control procedures.

·        Prepare food for therapeutic diets according to planned menus.

·        Prepare food according to standardized recipes and special diet orders.

·        Prepare and serve nutritional supplements and snacks.

·        Be sure that appropriate equipment and utensils are provided with the resident’s meal tray.

·        Review care plans to assure that current dietary information is consistent with food served.

·        Prepare and serve substitute foods to residents who refuse foods served.

·        Make only authorized food substitutions.

·        Assist in serving meals when necessary.

·        Assist in food preparation for special meals, parties, etc.

JOB REQUIREMENTS: 

 Education

·        Must possess, as a minimum, a tenth (10^th) grade education.

·        High School diploma or GED preferred.

 Experience           

·        Must have one (1) year of experience as a cook or in food preparation.

Experience in a health care facility is preferred
